*p.s. when I was in London last spring, I discovered there really IS a Duke Street! I wonder what the hymn tune name has to do with that, if anything...

What I've read is that the tune "Duke Street" is named after the street on which the composer John Hatton lived in Windle. Not an especially exciting explanation, but I guess as good as any other reason for a name.
